Authorization of Recurring Payment

By setting up your credit card payment, you hereby authorize University PC Care, Inc. to automatically charge the credit card on file for monthly or yearly recurring charges for the fees outlined in my original agreement including maintenance renewals. I understand that I can always request to cancel this authorization on-file by providing a written request via email (2) weeks prior to the effective date of this cancellation.
